Adult Home and Community Based Services

LIVINGSTON AND STEUBEN COUNTY RESIDENTS

HCBS (Home and Community Based Services) and CORE (Community Oriented Recovery and Empowerment Services) are provided to people enrolled in NYS Medicaid who have specific medical and mental health needs.  We help people in Livingston and Steuben County who are living with or in recovery from mental health or substance abuse disorders.  These services help people to be more self-sufficient.  The goal is to help individuals to make use of skills they already possess in order to support growth.  The most unique part is that all services are provided right in the member’s home or preferred community location.

People interested in getting more help in these areas of their lives need to be enrolled in a HARP (Health and Recovery Plan) by their MCO (Managed Care Organization).

Eligible individuals should contact their Health Home Care Manager for an assessment to be completed, which will further determine eligibility.  The individual has a choice of agencies which can provide HCBS.
