Adult Home and Community Based Services

LIVINGSTON AND STEUBEN COUNTY RESIDENTS

HCBS (Home and Community Based Services) and CORE (Community Oriented Recovery and Empowerment Services) are available to people enrolled in NYS Medicaid who have mental health, chronic medical needs, and/or need addiction & recovery supports. Members need to be enrolled in a HARP (Health and Recovery Plan) by their MCO (Managed Care Organization).

Providers: this can be determined by verifying their ePaces account has an H9 or H1 exemption code. Individuals/Families: this can be determined by your medical insurance enroller. Ask them to enroll you in a HARP plan.

We partner with Fidelis, Excellus, United Healthcare, and MVP insurance companies.

We help residents of Livingston or Steuben County, who are living with or in recovery from mental health or substance abuse disorders.  These services help people to be more self-sufficient and lead healthier lives.

Peers (those with lived experience) or Behavioral Health caseworkers assist individuals utilize skills they already possess or develop new ones to support growth and recovery.  Services are provided in the members’ home, workplace, or preferred community locations. These services are guided by the Individual Service Plan (ISP) created by the individual with their provider team. The ISP consists of goals and activities, which will help the individual reach their recovery and self-improvement goals.

For HCBS, an adult Health Home Care Manager will  help determine eligibility and make the referral.  The individual has a choice of which agency they prefer to work with in their local community.

For CORE, there is “no wrong door” for making a referral. Referrals can come from any source familiar with the individual, including self-referral. A medical necessity form (core-lpha-recommendation-form-fillable (1)) is required to be completed by a medical professional working with the individual.
