STEUBEN COUNTY RESIDENTS
Fresh Farmacy is a prescription/referral-based program for Medicaid recipients, available only through the Steuben County Turning Point service. Our staff works with you to:
- connected you with Primary Care physicians
- provide you with fresh fruits and vegetables for yourself and your household for six months at Turning Point locations
- qualify for food preparation classes
- receive assistance with other needs (transportation assistance to medical services, help with forms, etc.)
Am I eligible?
To participate, you must have Medicaid and a prescription from your doctor. Your doctor will make the recommendation based on your BMI and if you have a chronic health condition.
How can I enroll in this program?
You must have a prescription/referral from your primary care physician. Your primary care physician may refer you to the program, or a Turning Point staff member will help you get to your primary care provider in order to participate. Your primary care physician must be a participating provider in the program.
