The Family Support Worker will fulfill the home visiting function within the SHAPE program to include, weekly visits and crisis intervention in the client’s home environment. This may include schools, homes, clinics and other related sites. He/she is responsible for the completion of all Family Development Plans and other supporting documents; relates to community agencies, coordinating services as needed to facilitate family rehabilitation, reunification and child permanency.
