This position provides transitional case management services to aid in the reintegration of individuals being released from Livingston County Jail and residing in Livingston or Steuben County. Monitoring for up to 12 months post release with at least bi-weekly meetings to assist with clients “Transitional from Jail to Community” plans. Targeting referrals to employment programs, housing assistance, mental health, parenting skills, case management, client advocacy, collaboration with Livingston County Jail and Livingston County Probation.
